He sounds pretty selfish and manipulative the way you describe him. He might have more good sides to him than you write here for you to stay with him this long and to care for him the way you do. Either that or you have been massively manipulating you into doing what he wants and thinking that you enjoy it.
I really don't know what to tell you. Cheating is definitely wrong, but you had mentally checked out of the relationship before he became ill. That you stay with him despite that, well good for you, morally. But do you really owe him marriage in this situation? Can you make arrangements that you online there and are a caretaker as a friend but are not in a relationship with him? It sounds like your relationship was ending anyway.
You have problems communicating your desires, and that lies on you, not him. He won't know that you want oral sex, or that your relationship is unsatisfying for you if you do express it. You might want to try therapy for yourself to learn to assert yourself better. Good luck to you.
