I too have ADHD and have told him several times I think he has it and needs to talk to his doctor about it. He says he's fine.
I would be more empathetic about it if he tried to at least get tested, but since he refuses to do anything about it, I'm not going to feel bad for him. Because, again, I have it too, so I get it, but I get treatment and work on managing it whereas he just thinks it's no big deal

When i lost 10kg, people would never believe me because “where did you get it all out of?”. To some people and body types, 10kg is a lot, to others it doesn't look like much, but the next kg makes all the difference.

Excellent and totally logical answer. He may or may not be cheating on OP already. Or he may just have something going on in his head that relates to OP having become a mom. Either way, OP suggesting an open relationship or OP stepping outside the relationship will just open a massive can of worms. Better for OP to try relationship counseling like you suggested to try to figure out what is going on with her fiancé. The answer may be to break up and then become a single mom who can then look for a man that will give her the type of relationship that she wants longterm.
The vast majority of men who would engage OP in an open relationship will be there for sex, no emotional support, no caring for her if she gets seriously ill. Unfortunately for OP, her fiancé may be that type of man, there for the good times, but absent when challenges arise.
