I think everyone here is ignoring the fact that op does almost everything in the relationship. Maybe it’s not just about her appearance but her lack of effort overall? She barely cooks, cleans, works, and doesn’t wanna spend time with her husband. The weight might just be what broke the camels back for op which, without context can make him seem conceited but with everything else it’s understandable. It’s probably maddening to see someone who doesn’t do much not able to maintain themselves and become overweight (at that height she is overweight). If she doesn’t care about her health or being a good partner then there’s no reason to stay together. You can’t force someone to care.
Ask yourself this. Do you want to be in a similar relationship with him in five years time? What about for the next ten years or twenty years. If you’re happy with that thought then just stay. If not then you need to make some changes.

I should definitely do that. We do not have common marriage here but it's fine because they do recognize when couples present as if they are married combined with length of relationship and children we would have something similar to a divorce if we went that way. He would be fucked with child support alone because he does make really good money. So if I left I would be getting thousands a month just in that. If i wanted to leave so bad like another commenter said, I would. I don't want to and want to try this last time to make it work so I know I did my absolute best to keep my family together.
