Honestly, if you're on good terms with your family/friends, I would tell them; if husband can't be relied on (due to his disgusting remarks/attitude) then you will need all the support you can get, both before and after delivery. Personally, I know I'd want my siblings to reach out to me if they were suffering from such an abuse.
If you do not want him in the delivery room, make sure to have someone with you that you can trust and rely on to make medical decisions for you if heaven forbid something goes south. Plus, I'd also make sure the medical staff knows not to let him in the room (husband sounds like the kind of AH to ask the doctor to give you the 'husband stitch').
Please do not feel embarrassed. If anything he's the one who should be! By the sound of it, that is not the man you married, he has changed over time and not for the better.
