even if they love you alot and have no idea what harm they are doing, your parents' actions are endangering you. Make a little priority list . Your health and safety is at the top. Saving your parents from embarrassment is less important. If you can be safe and healthy (and your sister) without embarrassing your parents, fine. But if you can't, then don't worry about them until you have your health. Reach out to an adult on your medical/mental health team. Your parents may only be weird about weight, but it is a pretty important thing to be weird about. They need professional adults telling them how to parent on this one issue.
As a general thing, rubbing one out while in a relationship isn't a bad thing. More often than not partners will have different levels in libido and if one is too tired to go through the deed then as a courtesy the other will just want to take care of it just so they don't bother their partner. And it rings just as much truth if they, themself, are just too worn out from work and think of it as 'taking care of it themself'.
But if it really does bother you then I really would recommend to just sit down with him and let him know how you feel about it and find a middle ground
