You said you two moved across the country away from everybody he knows and loves, and you also said these hygiene issues didn't start until you moved… Sounds a whole lot like potential depression to me. Have you asked him if he's okay? He might need help (therapy, meds, etc).
Granted you should never feel like you have to parent your love interest, but start focusing on the ways he has changed… He might be dealing with a lot internally that he isn't talking to you about. Maybe communicate with him? Maybe he is the person you first got with, but it's now buried under all that emotional baggage that's causing all these changes?
Doesn't mean you need to stay with him, but he might have suddenly changed because he needs help.
